This is a tough one! We are fortunate to have lay leadership in this area; a couple who have been affected by addiction in their immediate family have led a support group supported by print material that has a specifically Christian focus (I can't remember the name of it) but addresses the addict and the family members, so all are welcome. I know not all churches can host something like that and my thought would be that with something like addiction you have to have a good network where you can refer them to--and then FOLLOW UP. They need to know that you still care!
